Three deaths at Hawke’s Bay Hospital

Hawke's Bay Hospital recorded three deaths that were deemed to be preventable during the 2010/2011 year, according to a report released by the Health Quality and Safety Commission into serious and sentinel events. Two of the deaths at the hospital were listed as falls. One fall resulted in a fractured hip and the patient died three days later. 40 medical mishaps include power-failure death

A patient on a ventilation machine at home who died because of a power failure is one of 40 medical mishaps recorded by the Southern District Health Board in 2010-11. Yesterday, the Government's Health Quality and Safety Commission released its serious and sentinel events report for 2010-11. Listed are 377 serious or sentinel events, with 86 deaths, although the deaths were not necessarily a result of the events. Read more [...]

‘End big four’s domination of listed audit market’ call

The commission is holding an inquiry into the dominance of KPMG, Deloitte, Ernst Young and Pricewaterhouse-Coopers (PwC) in examining the accounts of publicly traded companies. The big four audit all but one firm in the FTSE 100. BDO and Grant Thornton, the fifth and sixth biggest audit firms worldwide, and London-based Mazars, gave presentations to the inquiry last week, arguing that listed firms must be forced to regularly re-tender audit work to crack the big four's stranglehold of the market.
